The team has been put in place to work with patients of physicians who suffer from mild to moderate mental health issues. More complex cases are navigated towards the more appropriate community resources. All services are provided in the French language, however the interdisciplinary team and the COPD services are offered in both languages. Tied to the primary care team are other services such Diabetes education, foot care, falls prevention, seniors exercise classes and numerous other educational workshops. The services are provided in a newly constructed Centre that has become the hub of french language health services in the community.
